Each year, CNC's referral listing is distributed to over 5,000 local physicians, CNC members, and other groups seeking consulting nutritionists.  If you'd like to join CNC, print out the following application (on legal size paper), fill it out in its entirety and mail it with your payment to the address listed on the form.
